List of documents for applicants:

1. Statement
2. Educational document: original certificate or diploma + supplement to them (copy of the certificate only for boys).
3. Medical certificates:
– vaccination certificate (form 063) – 2 copies;
– medical certificate (form 086-U) – original (The conclusion should state “Can study and work in the chosen profession”) + 3 copies;
– проба Руф’є – оригінал;
– a certificate of registration (for boys in 11th grade) або 025Ю;
– health book (for the professions “Hairdresser. Manicurist. Pedicurist”, “Chef. Confectioner”, “Waiter. Bartender”, “Chef. Restaurant service master”);
– сертифікати або довідки від психолога та нарколога.
4. Photo card size 3x4 cm – 6 pcs.;
5. Copies of documents giving the right to benefits:
– children of miners;
– Chernobyl survivors;
– children of those who died in the line of duty;
– children of JFO participants;
– orphans;
– children deprived of parental care;
– half-orphans;
– children from large families;
– low-income;
– disabled children.

6. Document proving identity and citizenship:
– passport – 4 copies;
– certificate of registration of place of residence – 4 copies.
7. Identification number – 4 copies
8. Written consent for the processing of personal data (in accordance with the Law of Ukraine "On Personal Data Protection")
